Community Aged Care Packages - Northern Sydney
Community Aged Care Packages (CACP) support older people who wish to remain at home.
Each care package is personalised to your needs and can include the following services:
• Meal preparation
• Personal care
• Light housework
• Shopping
• Transport to appointments
• Outings
• Respite for carers
• Linking to other services, eg advocacy.
Who can access the service?
Any person who has been assessed as suitable by an Aged Care Assessment Team (ACAT). Your GP can refer you to the ACAT for your Local Government Area (LGA). Contact details are also available from your local public hospital.
We currently provide Community Aged Care Packages in the Hornsby, Ku-ring-gai,
Lane Cove, Manly, Mosman, North Sydney, Pittwater, Ryde, Waringah and Willoughby LGAs.
We also have staff to provide care and support to the Chinese and Italian aged communities.
UnitingCare Ageing offers a range of aged care services, including residential aged care, independent living, community aged care, day centres, private aged care nursing services and respite care.
We continually strive to develop innovative aged care services by seeking knowledge, responding to and meeting community expectations and by developing creative approaches that will provide the best possible care for the people we serve.
